This directory is brought to you by the combined efforts of the Hancock County Chronic Disease Self-Management Network—a big name for a small group of organizations that want to help you better manage your health. Listings in this directory originated with the Maine 2-1-1 system, which provides a larger database of state-wide resources. We have pulled out those that are related to managing chronic diseases, both disease specific and by prevention category. We have also focused on resources in Hancock County, and parts of Washington County. If you don't find what you're looking for here, try 2-1-1, which you can search online, or just dial 2-1-1 on your phone to talk to a helpful person. If you would like your organization, group, facility, or other resource listed here, simply click the "join us" tab above and fill out the"New Resource" form. We will include you in the My Health Finder database and we will forward your information to 2-1-1 so you can be included in their database, as well. Assess Your Health Risk Click on the Keep Me Well link below and answer the questions about your health. This will take you only 10-15 minutes to complete.
